Big Pump, Little Pump

It was a dark and stormy night...at the water treatment facility at a mid-west steel mill. Operators were sent out in the rain to tag out the big pump and they can't go home until they are done. Will they get the right pump? What happens if they don't? Does the organization help or hinder? Or, is anything that goes wrong the worker's fault? Let's talk about this scenario and discuss what could or should be done about it. How can we put HPT to work to help workers be more successful?
